Genny Savastano è ora potente e gestisce tutte le piú importanti piazze di spaccio di Secondigliano. È temuto e conosciuto come l’erede di don Pietro e si è ritagliato credibilità anche tra la malavita estera. Ha tradito inoltre il suocero don Giuseppe Avitabile mettendo il muso nei suoi affari sfruttando la sua detenzione ed il rapporto di parentela fra i due. Ciro invece non avendo più niente da perdere dopo la morte della figlia, uccide Malammore con l’aiuto di Genny e si ritira in Bulgaria come corriere al servizio di Valentin, malavitoso che opera a Sofia. Scoprendo di essere stato ingannato e di una taglia messa su di lui, Ciro uccide sia Valentin sia il figlio di lui, Mladen. Nel frattempo don Avitabile esce di prigione ed è sottoposto agli arresti domiciliari così si presenta a Genny ed Azzurra riallacciando i propri affari con il genero verso il quale nutre però forti sospetti arrivando a pensare che possa, con l’aiuto di Azzurra, aver testimoniato contro di lui facendolo arrestare, e scopre tramite Gegè che effettivamente Genny ha gestito molti suoi affari a suo nome vendendo in autonomia anche un carico di droga appartenente a lui. Con i suoi uomini più fidati, don Avitabile pesta e tortura Genny informandolo di aver inviato una lettera di denuncia anonima, di avergli sottratto parte dei suoi guadagni, di aver fatto uccidere un suo amico fornitore di droga in Honduras e soprattutto gli intima di non vedere mai più la moglie ed il figlio, che sotto il suo controllo potrebbe anche essere ucciso. Genny è sotto scacco e ha bisogno di alleati, che trova in Ciro e Scianel, desiderosa di ritornare al potere dopo essere uscita di prigione grazie alla ritrattazione della accuse di Marinella. Ciro quindi coinvolge nella lotta ai Confederati di don Avitabile anche l’ambizioso Enzo, detto Sangueblù, che aspira a Forcella e poi fino a Napoli Centrale grazie all’amico fraterno Valerio. Quest'ultimo è inoltre a capo di una storica cosca caduta in disgrazia e rifondata con membri più giovani. Grazie all'aiuto di Ciro, Sangueblù e Scianel (che finanzia le armi), Genny riesce a far ripartire gli affari con il traffico di droga e sfrutta la rielezione dell'amico Michele Casillo tramite l'assunzione di numerosi operai in aziende in crisi o fallite. ’o Sciarmante, membro dei Confederati, storica cosca del centro di Napoli in accordo con Avitabile, scopre peró il nuovo covo di Sangueblù e lo interroga per sapere chi c'è dietro i suoi affari proponendogli di passare dalla sua parte. Enzo non gli rivela nulla e rifiuta di accodarsi alla Confederazione. Una bomba carta lanciata al bar di Carmela, sorella di Sangueblù segna l'inizio della guerra fra i Confederati e Genny. Prima che la situazione degeneri Ciro propone alla Confederazione un armistizio che viene accettato. Enzo ottiene il controllo di Forcella e si dissocia da Genny. Savastano, però, ha bisogno del suo aiuto e di quello di Ciro per riavere la sua famiglia così uccide Carmela dando la colpa ad ’o Sciarmante, che dopo l'armistizio aveva ceduto Forcella ad Enzo, riportando il ragazzo dalla propria parte. Per riavere la sua famiglia Genny è costretto a cedere a tutte le richieste di Avitabile sulla gestione della droga ed Enzo a cedere nuovamente Forcella, covando odio e vendetta verso Avitabile ed i suoi uomini, ma il temibile Boss vuole anche la morte dello stesso Sangueblù e di Ciro. Scianel intanto passa dalla parte della Confederazione confidando anche nel doppio-gioco di Patrizia, che resta invece fedele a Genny e Ciro. Questi ultimi organizzano un agguato a don Giuseppe Avitabile ed ai suoi uomini che va a buon fine con Genny che può abbracciare la sua famiglia e lo stesso Avitabile che viene ucciso, così come ’o Sciarmante. A questo punto Ciro spinge Genny a chiedere la pace ad i Confederati, ora guidati da ’o Stregone, anziano malavitoso. La pace viene accettata ed Enzo Sangueblù ottiene nuovamente il dominio su Forcella. Patrizia uccide Scianel per i soprusi subiti da Marinella, amica di Patrizia, e come forma di biglietto da visita per la pace con Genny da parte degli altri Confederati. I fratelli Capaccio, ex componenti dei Confederati ma passati dalla parte di Sangueblù dopo la pace, uccidono ’o Stregone per non sottostare alle sue condizioni, ponendo fine ai Confederati. Invitati sullo yacht di Enzo per festeggiare la vittoria l'accordo, lui rivela le sue reali intenzioni, ovvero uccidere Genny. Ciro si sacrifica al suo posto dicendo di essere il mandante dell’omicidio di Carmela per proteggere l’amico. Genny è costretto ad uccidere Ciro ed a gettare il corpo in mare.

In the meantime Don Avitabile goes out of prison and is under house arrest so he shows up to Genny and Azzurra re-lending his business with his son-in-law to whom he nurtures strong suspicion thinking that he can, with the help of Azzurra, testify of him making him arrest, and he discovers through Gege that Genny has actually managed a lot of his business in his name by selling in autonomy even a load of drugs belonging to him. With his most trusted men, Don Avitabile crushed and tortured Genny, informing him that he had sent an anonymous letter of complaint, stole part of his earnings, killed a friend of his drug supplier in Honduras, and especially told him not to see never again his wife and son, who could also be killed under his control. Genny is in check and needs allies, which he finds in Cyrus and Scianel, eager to return to power after being released from prison due to the retraction of the accusations of Marinella. Ciro then involves the ambitious Enzo, called Sangueblù, who aspires to Forcella and then up to Napoli Centrale thanks to his fraternal friend Valerio in the fight against the Confederates of Don Avitabile. The latter is also the head of a historic cosca fallen into disgrace and refounded with younger members. Thanks to the help of Ciro, Sangueblù and Scianel (who finances the weapons), Genny manages to restart the business with drug trafficking and exploits the re-election of his friend Michele Casillo through the employment of numerous workers in companies in crisis or failed. 'O Sciarmante, a member of the Confederates, historical cosca of the center of Naples in agreement with Avitabile, but discovers the new den of Sangueblù and questions him to know who is behind his business by proposing to go over his side. Enzo reveals nothing to him and refuses to join the Confederation. A paper bomb thrown at the bar of Carmela, sister of Sangueblù marks the beginning of the war between the Confederates and Genny. Before the situation degenerates Cyrus proposes to the Confederation an armistice that is accepted. Enzo gains control of Forcella and dissociates himself from Genny. Savastano, however, needs his help and that of Cyrus to get his family back so kills Carmela blaming 'or Sciarmante, who had sold the Forcella to Enzo after the armistice, bringing the boy back to his side. To get his family back Genny is forced to cede to all requests for Avitabile on the management of drugs and Enzo to cede again Forcella, hatching hatred and revenge against Avitabile and his men, but the fearsome Boss also wants the death of the same Sangueblù and of Cyrus. Scianel meanwhile passes on the side of the Confederation also trusting in the double-game of Patrizia, which remains faithful to Genny and Ciro. The latter organize an ambush to Don Giuseppe Avitabile and his men who are successful with Genny who can embrace his family and the same Avitabile who is killed, as well as' Sciarmante. At this point Cyrus pushes Genny to ask for peace from the Confederates, now led by 'o Sorcerer, an old criminal. Peace is accepted and Enzo Sangueblù once again gains dominion over Forcella. Patrizia kills Scianel for the abuses suffered by Marinella, a friend of Patrizia, and as a form of visiting card for peace with Genny by the other Confederates. The Capaccio brothers, former members of the Confederates but passed by the side of Sangueblù after the peace, killed 'or Sorcerer not to submit to its conditions, putting an end to the Confederates. Invited on Enzo's yacht to celebrate the victory the deal, he reveals his real intentions, namely to kill Genny. Ciro sacrifices himself in his place by saying that he is the instigator of the murder of Carmela to protect his friend. Genny is forced to kill Cyrus and throw his body into the sea.
