Il comico fallito Arthur Fleck ammazza il presentatore televisivo Murray Franklin in diretta e deve scontare la sua pena nel manicomio criminale Arkham State Hospital. Qui incontra Harleen Quinzel e i due si innamorano perdutamente, vivendo insieme un'allucinazione musicale fomentata dai loro deliri condivisi. Nel frattempo, i seguaci di Fleck all'esterno del manicomio si uniscono in un movimento di protesta per far tornare in libertà il loro leader spirituale. Sono i prodromi di quello che diventerà l'impero criminale del Joker.
