McMurphy has a criminal past and has once again gotten himself into trouble and is sentenced by the court. To escape labor duties in prison, McMurphy pleads insanity and is sent to a ward for the mentally unstable. Once here, McMurphy both endures and stands witness to the abuse and degradation of the oppressive Nurse Ratched, who gains superiority and power through the flaws of the other inmates. McMurphy and the other inmates band together to make a rebellious stance against the atrocious Nurse.

Randle McMurphy, arrestato per piccoli reati, viene portato in una clinica psichiatrica perché tenta di fingersi pazzo per sfuggire al carcere.
Qui viene a contatto con gli altri pazienti dei quali diventa ben presto il beniamino:
si prende gioco delle sedute di psicoanalisi, si improvvisa telecronista, organizza una piccola "gita" in barca.
L'ospedale è diretto da una ferrea capoinfermiera, la signorina Ratched. McMurphy decide di fuggire.
Con lui è un gigantesco indiano che si è sempre finto sordomuto. Prima di fuggire però McMurphy organizza una festicciola introducendo due ragazze tra i malati.
Un Nicholson in splendida forma dà corpo e anima a questo outsider, eroe suo malgrado.
Il film si regge in fondo su di lui e soprattutto sulla sua contrapposizione all'infermiera, una grande Fletcher dalla recitazione tagliente.
Forman tocca alcune corde dell'emotività facendo leva sui sentimenti più elementari.
Tratto dal romanzo omonimo di Ken Kesey, il film vinse cinque premi Oscar:
